New Client Appointment Request

Please complete the form below to request a new client appointment with South Shore Animal Services. Be sure to include your pet’s medical and vaccination records with your submission so we can review their history and complete the registration process.

Submitting your pet’s medical and vaccination records with this form will help speed up the registration process and avoid delays in getting your pet ready to schedule.

Please note: Invoices and receipts are not considered medical records. We need your pet’s actual medical records, including treatment history, vaccination history with dates administered, and Rabies certificate.

Once we receive your information, we will set up you and your pet in our system. When the registration process is complete and we are ready to schedule your appointment, you will receive an email with instructions to contact our office to schedule. Please allow 24–48 business hours for registration processing.

Submitting this form does not schedule an appointment. You will receive an email from us when you and your pet are fully registered and ready to schedule.
